Communicates across multiple service lines to help facilitate appointments, outpatient procedures and follow-up care. Assists with prior authorizations as required by insurance companies. Assesses, plan, and evaluate the needs of the patient in collaboration with the Hospitalist and will communicate with providers, case manager, nursing staff and entire multi-disciplinary care team to support requested services. Serves as a point of contact for patients, specialties, primary care providers, and other outpatient services if questions arise after discharge.

Will perform all job duties or job tasks as assigned. Will follow and adhere to all requirements, regulations and procedures of any licensing board or agency. Must comply with all Benefis Health System's organization policies and procedures.
Education/License/Experience Requirements:- Graduate of an accredited school of nursing
- Current state Registered Nurse Licensure, within state of practice.
