Position Summary: Administers patient care in an inpatient or observation setting where an acute care patient receives active treatment for injuries, illnesses, medical conditions, or post-intervention assessments.
Responsibilities
Essential Functions:
- Assesses patient needs and develops/revises individualized care plans based on responses; evaluates progress towards expected outcomes.

- Serves as preceptor, charge nurse, educator, and/or nurse clinician.

Hospital Care at Home (Program Functions - Patient Care Hub)
Provides patient care virtually, assessing needs and developing care plans using TeleHealth technology, collaborating with in-home staff, and addressing age-specific needs.
Hospital Care at Home (Program Functions - In-home Nursing)
Delivers direct care in patients' homes, assessing needs, collaborating with virtual teams, and managing urgent/emergent issues. Must be able to travel within designated regions, use virtual technology for visits, and transport items as needed.
Qualifications
Education/Training: Graduate of an approved nursing school; meets competency requirements.
Licensure/Certification: Valid Alabama RN license or multistate license, BLS, and other area-specific certifications (ACLS, NRP, PALS, TNCC). Valid driver's license and reliable transportation required.
Experience: Minimum 1 year of nursing experience; 2 years for Hospital Care at Home role, preferably in acute care settings.
