Hours: Primarily Monday through Friday daytime hours with opportunities to have a flexible schedule including remote work.
General Description of the Position:
Supports a case load of adult participants with varied intellectual developmental disabilities who reside in groups homes, apartments and homes within the Rapid City community. Facilitates medical services through the Direct Support Professionals who work in the participant's home and are non medical personnel. This position is a vital role in the participant's team providing medical advice, training and support.
Possesses the nursing knowledge and experience in working with participants with varied intellectual developmental disabilities. Facilitates initial and ongoing assessments on the following: nutrition, pharmacy, plan of care, infection control, medical history, level of care certification, nursing care reports, medication, laboratory values and psychosocial issues. Addresses emergency needs in the residential homes as they arise. Presents a professional image to the agency staff, general public and to the medical community.
Qualifications:
Current nursing licensure in South Dakota.
Current BLS Certification
Valid driver's license.

Primary Responsibilities:
Provides information and assists team in developing programs to enhance an individual's community integration.
Provides consistent communication with medical providers to advocate and follow up with medical concerns.
Provides training and consultation to non-medical personnel.
Completes infection Control Program for the agency, maintaining logs and making recommendations as required to assure high standards of safety.
Confirms upcoming appointments and refers to MSA (Medical Support Assistant) to schedule.
Attends specialty medical appointments as needed.
Completes appointment medical data forms and inputs into computer record and carries out doctor's orders.
Maintains current record of all health-related appointments on each participant in his/her caseload in a nursing medical file.
Accepts and documents physician orders.
Documents medication, dietary, treatment and all health-related orders and assures that the participant, staff, and guardians or other caregivers are aware of new or changed interventions.
Performs medical assessments and addresses emergency needs as they arise.
Performs IM injections, Foley catheter insertion, supra pubic catheter changes and non delegatable wound care for participants as required per certified medical professional orders.
Maintains strict confidentiality of health information.
Provides staff training to assure health needs of participants are met such as medication administration certification classes and others.
