JOB SUMMARY
• The CHF Nurse Navigator is responsible for coordinating the individualized care for patients diagnosed with ongoing critical cardiac health conditions by serving as an advocate, educator, consultant, leader, and community ambassador for the specific patient population served in accordance with Phoebe Putney Health System strategy, vision, and values, as well as acceptable clinical practice and requirements for quality. The CHF RN is a consistent care coordinator throughout the continuum of care and can assess the physical, psychological, and social needs of the patient. Work is strongly focused on 1) guiding the patient/family through the complex healthcare system, working to eliminate barriers that might otherwise adversely impact patient care/outcomes; and 2) helping to coordinate the efforts of the medical team to facilitate continuity of care and enhance patient satisfaction. The CHF Nurse Navigator is also accountable for keeping abreast of new/changing regulatory requirements impacting matters within the designated scope of responsibility, maintaining current knowledge of hospital and outpatient policies/standards/procedures, DNV Standards, new clinical procedures/processes, patient safety goals, core measure quality indicators and other requirements impacting matters within assigned scope of responsibility. The objective of the CHF Nurse Navigator is to help the patient become compliant with management of their chronic cardiac disease which in turn will improve the patient's quality of life and reduce the risk of a readmission in less than 30 days. The CHF Nurse Navigator works in collaboration with patients, nurses, physicians, and therapists to ensure the patients have a thorough knowledge of their disease, the purpose and use of medications, and available support services (Examples: Cardiac Rehab, Phoebe Putney Memorial Hospital Staff, Home Health, Mobile Integrated Health, Support Groups such as Mended Hearts, etc.) and to ensure the patients have what they need in their home after discharge. The CHF Nurse Navigator will provide post-discharge support to patients via phone calls and follow a systematic approach to visiting this patient population Monday through Friday. All activities will be performed in support of the strategy, vision, and values of Phoebe. • Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
• Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.
• Wears protective clothing and equipment as appropriate.
• Must possess and maintain a valid Georgia Driver's License and meet auto insurability requirements as approved by Phoebe Putney Health System's insurance company. WORKING CONDITIONS
• General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.
• May be exposed to high noise levels and bright lights.
• May be exposed to limited hazardous substances or body fluids, or infectious organisms.
• May be required to change from one task to another of different nature without loss of efficiency or composure.
• Periods of high stress and fluctuating workloads may occur.
• Long-distance or air travel as needed - not to exceed 10% travel.
Qualifications
EDUCATION REQUIREMENTS
• 2 year / Associate Degree in Nursing (Required) EXPERIENCE REQUIREMENTS
• 2 years nursing and experience in Cardiology (Required) CERTIFICATIONS AND LICENSURES
• Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license, Basic Life Support (BLS), Obtain Nurse Navigator Certification in Congestive Heart Failure within 12 months of employment (Required) GENERAL SKILLS
