Registered Nurse

We are hiring RN Case Managers with a sign-on bonus for full-time roles focused on hospice care.
•  Provide patient-centered, home-based care by developing and reassessing care plans.
•  Coordinate with interdisciplinary team members including social workers, spiritual care, and medical directors to ensure comprehensive needs are met.
•  Supervise nursing staff and aides while communicating patient status and collaborating with providers.
•  Offer competitive pay ($65k-$85k), benefits (health, dental, vision, 401(k , flexible schedules, company vehicles, and ongoing education.
•  Must hold an active PA RN license, have at least 1 year of hospice experience, and be willing to relocate to Wexford, PA.
•  Sign-on bonus options include up to $15,000 over three years.
