Registered Nurse

We are seeking an RN Case Manager in Troy, MI to coordinate and deliver hospice care in patients' homes and communities.

•  Conducts comprehensive assessments, develops personalized care plans, and evaluates patient status regularly.

•  Provides nursing care, administers medications, educates patients and families, and collaborates with interdisciplinary teams.

•  Maintains detailed clinical documentation, communicates with physicians, and ensures safety and continuity of care.

•  Participates in on-call duties, supervises auxiliary staff, and engages in ongoing professional development.

•  Requires RN licensure, relevant experience, strong communication, and ability to perform physical tasks.
