Core Responsibilities:
- Perform home and facility visits to support patients and families
- Complete admissions, re-certifications, assessments, and confirmation of death visits
- Collaborate with interdisciplinary teams to ensure quality end-of-life care
- Maintain accurate and timely documentation
What You Bring:
- Current RN license in the state of Indiana
- Previous hospice or home health experience preferred
- Strong clinical judgment, communication skills, and compassion
- Ability to work independently while managing a small, focused caseload
