Provide direct skilled nursing care to terminally ill patients following the care plan under RN supervision

Assess patient/family needs, administer medications and treatments, and document clinical notes

Monitor and report patient condition changes

Support patients and families with education, comfort, and self-care guidance

Must be a licensed LPN/LVN with at least one year of relevant experience
