Registered Nurse-Long Term Care

Position Purpose: The Registered Nurse is responsible for educating, assisting, and interacting with patients in an outpatient clinic setting with regards to health promotion and wellness. Key Functions and Responsibilities The Long-Term Care registered nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age- specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, provides family education. The long-term care nurse shall be responsible for patient ADLs (activities of daily living), assessments, BCMA medication and procedures per established policies and guidelines (ie). Barcode medication and administration, IV therapy, blood transfusion, monitor blood pressure, weight and hydration, administration tube feeding, management of behavioral patients, interdisciplinary team participation, admissions, and discharges, internal and external transfers, delegating to ancillary staff, electronic medical record documentation, patient transport, and emergency response procedures. Qualifications Required Education and Licensing. Current unrestricted license. BLS (American Heart Association approved) Required Knowledge and Experience. Minimum of three years recent experience in acute care or other areas. High-level of reliability and dependability A valid driver's license is required. Working Conditions and Environment This position is on-site in a medical facility. Driving may be required. Physical Requirements The position requires repetitive motion, such as standing, walking, sitting and bending. Light lifting, up to 20 pounds, may be required.
