Job Summary
Responsibility for internal referral coordination and intake procession of patient's being referred from institutional facilities and clinics. Acts as a liaison between hospitals, medical offices, home health agencies, and medical suppliers. Works under the direct and indirect supervision of the Director for Homecare Coordination.
Essential Functions

Evaluates patient data for appropriateness of homecare services. Completes required forms and documents per agency/hospital policy and procedures.
Keeps Homecare staff informed of possible referrals from hospital. Coordinates patient's admission to Homecare agencies.
Assesses patients to initiate appropriate referrals and specialty services. Communicates and implements change appropriately. Explains homecare services to patient/family/caregiver including admission criteria.
Communicates with staff involved in patient's care (may include physician, social worker, nurse, therapist.) Acts as an educational resource for other team members. Acts as a liaison with physicians, hospital, insurance, medical equipment companies and clinical areas.
: Demonstrates active participation as a liaison with physician's office, hospital and medical support services. Assists with orientation and training seminars, for agency staff and community education programs. Endeavors to work well with team members and hospital staff. Complies with hospital and agency policies addressing safe working condition, recognizes inappropriate patient care referrals and refers to immediate supervisor. Completes tasks specified as leadership role responsibilities with appropriate documentation. Willingly accepts assignments outside of the normal duties of the position when such fulfills a special need within the agency.
Performs other duties as assigned.
Supervisory/Management Responsibilities
Minimum Requirements
Education - Graduate of an accredited school of Nursing program and approved by the South Carolina State Board of Nursing. Bachelor's degree in Nursing preferred.
Experience - Experience in Home Health nursing.
In Lieu Of
Required Certifications, Registrations, Licenses
RN currently licensed in South Carolina. Presents credentials which demonstrate ability to meet performance criteria for RN.
Valid driver's license, an acceptable three-year motor vehicle records as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance. Driving is required for this position using the employee's personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Driver Safety Policy and specific department driving policies. Employee must pass driver training.
