The RN works within the clinical care team to assure quality patient care and to support the goals of Lake Superior Community Health Center. JOB QUALIFICATIONS AND CREDENTIALS Associates or Bachelors Degree in Nursing from an accredited School of Nursing. Registered nurse with a current license in Minnesota and Wisconsin. One to three years in ambulatory care. Able to provide health care in a sensitive, nonjudgmental manner to persons of all economic levels and cultural backgrounds. Capable of independent nursing assessment decisions and nursing management in a primary care setting. Computer experience. Willing to work flexible hours, including evenings, as required by position. Current CPR certification or ability to become certified within six months from date of hire. JOB KNOWLEDGE, SKILLS, AND ABILITIES Ability to work efficiently and productively at all times. Ability to understand and follow written and verbal instructions. PRIMARY RESPONSIBILITIES Serve as a positive, enthusiastic, professional and proactive team member of the clinical team. Assure the provider workflow is protected through effective triage of issues which might impede efficiency. 3.Coordinate pre-visit planning for patients including review of routine labs, preventative services, immunizations, and acquiring supporting documentation from other providers. 4.Participate and coordinate as needed team huddles on a daily basis to discuss the daily schedule and anticipate needs for the day: 4.1 Identify patients on the schedule who may require more time or need a concurrent RN appointment to provide additional education outside of the provider appointment. 4.2 Assure all consents and needed equipment are readily available for procedures 4.3 Identify and mitigate unexpected staff shortages. 4.4 Review schedule for openings. 4.5 Identify patients on the schedule who have a history of frequent appointment no-shows. 4.6 Assure needed documentation is present such as controlled substance agreements, hospital follow-up, specialty, diagnostic or lab reports, etc. Provide direct care to patients and assists health providers with clinical procedures consistent with protocols. 6.Assure patient flow is efficient, providers are on-time, and patients are redistributed as necessary in order to stay on-time. Provide oversight and direction to the medical assistants. Triage walk-in patients and schedule them based on acuity and provider availability. Provide instruction and education to patients and their families. 10.Manage prescription refill requests for medications which are not controlled substances. 11.Triage telephone calls in order to determine patient's health care needs and make a plan on how to best meet those needs. Assure call backs are made by the end of the work day. 12.Document all care and patient interactions, including phone calls, in the medical record according to policy. 13.Coordinate services for patients and families utilizing other health related agencies and resources; obtains referrals to other physicians and specialists. 14.Coordinate immunizations including determining patient vaccine needs, vaccine inventory/storage, record keeping/tracking, and changes in immunization recommendations. 15.Assist with the acquisition of medications and supplies, including indigent programs for medication acquisition.
