A healthcare organization in New York City is seeking a licensed Registered Nurse (RN) to join their growing Community Health team in the Bronx.
About the Opportunity:
- Schedule: Tuesday to Saturday or Sunday to Thursday
- Hours: 8:30am to 5:00pm
- Setting: Field
- Visits: 4-5 per day
Responsibilities
- Conduct in-home and community-based clinical assessments using the nursing process
- Complete and manage UAS-NY assessments and ensure compliance with regulatory standards
- Collaborate with the interdisciplinary team to develop and update personalized care plans
- Administer medications, perform skilled nursing procedures, and monitor patient responses
- Educate members and caregivers on medications, self-care, and health maintenance
- Coordinate care across various settings, including hospitals, SNFs, etc.
- Document all care and assessments accurately and timely
Qualifications:
- 1+ year of Clinical Nursing experience (CHHA, Acute Care, Medical-Surgical, and/or Critical Care)
- Graduate of an NLN-accredited Nursing program
- Current NYS Registered Nurse (RN) license
- Strong communication, clinical judgment, and documentation skills
Desired Skills:
- Bachelor's Degree in Nursing
- Experience with Frail or Elderly populations (training provided if needed)
