Pay Median: $70.00/hr or $2625/week - $86.8/hr or $3300/week(For 37.5 hrs)

Role Description :
The Registered Nurse will be expected to have knowledge of discharge planning and make referrals. The RN will typically work on a Med-Surg or Tele unit and collaborate with a Social Worker. The nurse must be able to function independently and work as part of an interdisciplinary team, including physicians, for discharge planning. This position involves rounding with the team and contributing to discharge decisions for patients.

Key Responsibilities:
Manage a caseload of 17-25 inpatient cases (Med-Surg/Tele units)
Assess patients and develop individualized discharge plans
Coordinate care transitions to home, SNF, rehab, or shelter
Collaborate with physicians, social workers, and the care team
Participate in daily interdisciplinary rounds
Arrange services like home health, dialysis, wound care (KCI), etc.
Utilize systems such as CarePort, MCG, or InterQual for documentation
Perform PRI or SCREEN assessments as needed
Support Utilization Review and ensure proper level-of-care placement
Educate patients and families on discharge instructions and follow-up
Ensure compliance with hospital policies and patient safety standards

Shifts & Scheduling :
Monday-Friday, 8:00 AM - 4:30 PM (7.5 hours with a 1-hour lunch)

Required Qualifications:
Knowledge of CarePort, MCG, or InterQual is preferred.
Prior experience in Utilization Review (UR) is a plus.
Must be able to handle at least 20 patients on an inpatient unit.
RN License of NY State
AHA - BLS
PRI (Patient Review Instrument) Certification or SCREEN Training Certification
COVID Card

Additional Notes :
The RN will be expected to work collaboratively and efficiently within the interdisciplinary team, handling cases across various medical specialties.
