Schedule: 1.0 FTE (80 hours per pay period, Exempt) Monday-Friday hours.

We are excited to invite a highly motivated and experienced RN Program Manager to lead our Large Case and Catastrophic Care Management initiatives.


Position Overview: As the RN Program Manager, you will play a pivotal role in managing large case and catastrophic care management. Your responsibilities will include proactively identifying large case members, monitoring large case rosters, referring patients to existing care management (CM) programs, managing transplants and End-Stage Renal Disease (ESRD), and coordinating with specialty networks.


Key Responsibilities:


  • Utilize data analytics and clinical insights to identify members with large case and catastrophic care needs.
  • Maintain and update rosters of large case members, ensuring accurate and timely information.
  • Refer patients to appropriate existing care management programs to ensure comprehensive and coordinated care.
  • Oversee the management of transplant cases and ESRD patients, ensuring they receive the necessary care and support.
  • Collaborate with specialty networks to ensure seamless care coordination and optimal patient outcomes.
  • Create and implement individualized care plans for large case members, focusing on their unique needs and conditions.
  • Track and evaluate patient progress, adjusting care plans as needed to achieve desired outcomes.
  • Work closely with primary care physicians, specialists, and other healthcare providers to ensure cohesive and integrated care.
  • Ensure all care management activities comply with regulatory requirements and organizational policies.

Req;uired Qualifications:

  • Education: Bachelor's of Nursing Degree (BSN)
  • Experience: 5 years of RN experience in specialty area
  • Licenses/Certifications: Current RN License from State of Practice (Idaho)

Preferred Experience and Skills:


  • Strong Analytical, Organizational, and Communication Skills: Ability to analyze complex data, organize tasks efficiently, and communicate effectively with patients, families, and healthcare providers.
  • Data Analytics and Care Management Software Experience: Proficiency in using data analytics tools and care management software to identify high-risk patients and manage care plans.
  • Care/Case Management Experience: Extensive experience in care or case management, particularly with large case and catastrophic care, including transplants, ESRD, dialysis, and other complex conditions.
  • Utilization Management Experience: Knowledge and experience in utilization management to ensure appropriate use of healthcare resources and services.
