We are seeking a compassionate and dedicated Hospice RN Case Manager, (RN) to join our team near Bedford, NH. The ideal candidate will provide high-quality nursing care to patients in a hospice setting , ensuring comfort and dignity during end-of-life care. This role requires a strong understanding of geriatric care and the ability to work collaboratively with interdisciplinary teams to support patients and their families.

Schedule and Contract Details:
Position: Hospice RN Case Manager
Start Date: 4/21/2025 (or sooner if compliant)
Schedule: Monday through Friday, 8-hour shifts
On Call: No weekends, but may need to cover back-up on call
Territory: Bedford/Nashua/Manchester/Milford/Peterborough/Jaffrey areas
Contract Length: 13 weeks, with multiple potential extensions
Compensation: Up to $2000/week + mileage reimbursement

Responsibilities:
Conduct comprehensive physical examinations and assessments of patients in hospice care.
Develop and implement individualized care plans that address the unique needs of each patient.
Administer medications, including tube feeding, as prescribed by physicians.
Monitor patient conditions and provide triage as necessary, ensuring timely interventions.
Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care.
Educate patients and their families about disease processes, treatment options, and end-of-life care.
Maintain accurate documentation using electronic health record systems such as HCHB (HomeCare HomeBase).

Qualifications:
Valid Registered Nurse (RN) license in the state of New Hampshire
1+ Year Recent Experience in hospice and geriatrics in a home setting
Must be willing and able to complete admissions
Proficiency with HomeCare HomeBase (HCHB) REQUIRED
Experience in developing care plans and providing compassionate end-of-life care.
