Compensation: $54.00 - $60.00 per hour (DOE)

About the Role
An established and mission-driven healthcare provider is seeking a Registered Nurse Case Manager (RNCM) to join its hospice care team in San Diego. This full-time position operates Monday through Friday and covers North County (inland and coastal) and East County San Diego areas.
Key Responsibilities
Plan, coordinate, and deliver hospice care services in patients' homes
Conduct assessments and develop individualized care plans in alignment with hospice philosophy
Monitor patient progress and communicate regularly with physicians regarding condition changes and care needs
Provide patient and family education on disease process, medications, and self-care
Administer prescribed treatments and pain/symptom management
Collaborate with the interdisciplinary team, including attending and contributing to care conferences
Supervise hospice aides and ensure cohesive delivery of services
Maintain timely, accurate documentation in compliance with standards and regulations
Qualifications
Graduate of an accredited nursing program
Active RN license in the State of California
1-2 years of recent acute care experience in an institutional setting
1-2 years of recent home care experience
BLS Hands-On CPR certification (current)
Bachelor's degree in Nursing preferred
Hospice care experience preferred
Strong communication, organizational, and critical thinking skills
Physically able to lift and assist patients; mobility required for in-home care
Valid driver's license and insured, reliable vehicle
