As an RN Case Manager, you will oversee medical support services in 24/7 LPN-staffed homes and for individuals needing nursing care in non-medical settings. Key responsibilities include:
Conducting RN assessments and developing individualized Nursing Care Plans (NCP). Training non-licensed staff on emergency protocols and delegating nursing tasks per OSBN rules. Monitoring and evaluating medical needs and interventions. Advocating for the health and welfare of individuals supported. Collaborating with healthcare providers and families. Qualifications include a valid Oregon RN license, one year of medical/surgical nursing experience, and knowledge of relevant administrative rules.
