As an RN Case Manager, you will oversee medical support services in 24/7 LPN-staffed homes and for individuals needing nursing care in non-medical settings. Key responsibilities include:
Conducting RN assessments and developing individualized Nursing Care Plans (NCP). Training non-licensed staff on emergency protocols and delegating nursing tasks per OSBN rules. Monitoring medical needs and ensuring timely completion of medical appointments. Advocating for the health and welfare of individuals supported.
