Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. This role is essential in providing high-quality patient care through effective case management practices. The ideal candidate will possess strong clinical skills and a thorough understanding of healthcare systems, including experience in behavioral health, pediatrics, geriatrics, and primary care settings. The Registered Nurse Case Manager will work collaboratively with patients, families, and healthcare providers to ensure optimal health outcomes while adhering to HIPAA regulations.
Duties
Conduct comprehensive assessments of patients' medical histories and current health status.
Develop and implement individualized care plans based on patient needs and preferences.
Coordinate care across various settings, including clinics and hospitals, to ensure continuity of services.
Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
Educate patients and their families about health conditions, treatment options, and self-management strategies.
Maintain accurate medical records and documentation in compliance with HIPAA regulations.
Collaborate with interdisciplinary teams to address complex patient needs effectively.
Advocate for patients to access necessary resources and services within the healthcare system.
Requirements
Current Registered Nurse (RN) license in the state of practice.
Experience in case management or related nursing fields such as behavioral health, pediatrics, geriatrics, or primary care preferred.
Strong knowledge of medical records management and HIPAA compliance.
Excellent communication skills with the ability to build rapport with patients and families.
Ability to work independently as well as part of a collaborative healthcare team.
Strong organizational skills with attention to detail in managing multiple cases simultaneously.
Proficient in using electronic health record (EHR) systems for documentation and data management.

Pay: $2,328.00 - $2,709.00 per week
