RN Case Manager Home Visits, Grand Rapids Area
Join Our Team as an RN Case Manager for Home Visits in the Grand Rapids Area. Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Team.
This position involves providing direct care to clients within a 35-mile radius of Grand Rapids.
Currently, our greatest need is the North and Southwest Territory.
The North territory includes Rockford, Greenville, Cedar Springs, and some parts of Sand Lake
The Southeast territory includes Lowell, Ada, Alto, and Caledonia.
The Southwest territory includes Allendale, Hudsonville, Byron Center, Grandville, Wyoming.
Responsibilities
Perform initial and comprehensive assessments and deliver compassionate care to clients
Collaborate with physicians and other care team members to develop a Plan of Care
Promotes client and family/caregiver independence through counseling and education
Coordinate client discharge planning
Identify barriers to desired outcomes and evaluates outcomes of care
Prepare clinical documentation, progress notes in a timely manner using iPads for the EMR system
Participates in on-call rotation
Location: Grand Rapids, covering a 35-mile radius around GR
Schedule/Shift
Monday - Friday; Day shift 8-5pm, with flexibility in scheduling to help you maintain a healthy work/life balance
On-call rotation every 8-10 weeks to cover PRN visits, on-call triage and weekend admissions, etc
Compensation
Our compensation model includes bonuses for high-quality client outcomes and efficient case management
On call pay
Mileage reimbursement
Premium pay for weekend & holiday visits
Qualifications
Licensed RN in Michigan; BSN preferred
Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits
1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred
Passion for delivering high quality care to older adults
