Job Details

What You Will Be Doing

Conducting comprehensive care manager assessments
Collaborating on individualized patient-centered care plans
Utilizing tools to identify and manage patient risks
Coordinating tasks and interventions to support care plans
Facilitating care transitions across healthcare settings
Coordinating care management tasks with the healthcare team
Communicating with payers to optimize reimbursement and approvals

Skills You Need

Graduation from a state accredited school of professional nursing
Registered Nurse with a valid license to practice in North Carolina
Minimum of 2 years of experience as a registered nurse
Strong knowledge of risk segmentation, patient assessments, and care planning
