Care Partners is seeking a full-time Registered Nurse for the position of RN Case Manager to provide coverage in the Portland metro area. This position has candidates in the field providing direct patient care.

The onboarding and training process will involve hands-on learning with an RN preceptor, directly supported by our Clinical Managers and will be tailored to your level of experience until you feel comfortable working independently

Your Responsibilities (Includes but Not Limited to):
Provide skilled nursing assessment and planning and coordination/care management across settings to patients in private homes, adult foster homes, residential/assisted living care, and/or nursing facilities.
Maintain up-to-date patient records, including recording problems, plans, actions, and goals accurately and clearly reflective of changes as they occur in accordance with policies and procedures.
Provide nursing care, attentive to physical, emotional, and spiritual needs of patients.
Counsel patients and families regarding symptom control, medications, direct nursing care measures, nutrition, etc.
Assist patients and families in making choices that best meet their needs by providing alternatives and support.
Develop and implement a plan of care for patient/family/ and/or caregivers in consultation with other interdisciplinary group members (IDT).
Communicate often and well with other Care Partners staff; function as an integral member of the patient care team.
Make visits at death, support families, assist with final arrangements.
Educate families, arrange services, order equipment, obtain physician orders, and order medications.
Collaborate professionally with other health care providers, including the patient's physician, pharmacies, and care facilities to ensure optimal care for patient.
Qualifications:
Requires current unencumbered Oregon licensure as a Registered Nurse prior to first day of employment.
One year of professional nursing experience in hospice, palliative care, home health, medical surgical care, or equivalent.
Must have a valid driver's license, valid auto insurance, acceptable driving record and have access to a reliable automobile to travel to and from patient homes.
Requires well-developed critical thinking, patient assessment, and problem-solving skills.
Requires the ability to work independently and as part of an interdisciplinary team with strong verbal and written communication skills.
Must have good skills in using electronic tools such as email, internet, keyboarding, and be able to learn medical software to use for accurate and timely charting.
Bi-lingual, Spanish-speaking ability and certification in Hospice and Palliative Care are helpful but not required.
Requires basic life support CPR card or ability to obtain a valid card prior to patient contact within two weeks of hire.
Preferred applicant will be required to pass a background check and pre-employment drug screen, following an offer of employment.
New employees are required to complete tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
