ZoomCare is seeking a Registered Nurse Care Manager to join our team. The RN Care Manager is responsible for identifying and monitoring patients in need of care management, conducting post-visit follow-up calls for patients who have experienced hospital or emergency department visits, and providing episodic care coordination. The RN Care Manager is essential in ensuring our patients receive the comprehensive support they need to navigate their healthcare journey effectively.

  • Identify patients requiring care management based on clinical data and referrals.
  • Monitor patients' health status and adapt care plans as necessary.
  • Conduct follow-up calls with patients post-hospitalization to assess recovery and ensure care continuity.
  • Educate patients on discharge instructions and necessary follow-up appointments or medications.
  • Coordinate care among providers, specialists, and community resources to ensure comprehensive management.
  • Develop and implement individualized care plans in collaboration with the healthcare team and patients.
  • Maintain accurate patient records, documenting care management activities and assessments.
  • Analyze patient data to track outcomes and identify areas for improvement.
  • Provide education and resources to patients and families on disease management and preventive care.
  • Foster relationships with patients to support their engagement in their care.
  • Collaborate with interdisciplinary teams to optimize patient care.
  • Participate in team meetings and case reviews to discuss patient progress and care plan adjustments.
  • Work with peers and managers to develop and enhance the RN Care Management role over time.
  • Other duties as assigned.

QUALIFICATIONS

  • This is a remote position but you must be located in Oregon or Washington state.
  • Current RN licensure in Oregon and Washington.
  • Associate's degree in Nursing (ASN) required; Bachelor's degree in Nursing (BSN) preferred.
  • 2+ years of clinical nursing experience required; care management or case management experience preferred.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with patients and families.
  • Ability to maintain patient privacy and confidentiality.
  • Proficiency in electronic health records (EHR).

  • Salary Range: $105,000 - $120,000 dependent on experience

WORKING CONDITIONS

  • Project timelines and work volume/deadlines may often require more than the scheduled hours per week or work outside of regular business hours to complete essential duties of this job.
  • Prolonged periods of standing/sitting and working at a computer.
  • Occasional travel, approximately 4-6 times per year, to the Portland and Seattle areas for meetings with strategic network partners.
  • Must wear a mask as needed
