Job Summary

Benton County is seeking a Registered Nurse Care Coordinator for the School-Based Lincoln Health Center.

JOB SUMMARY
Registered Nurses work in clinical, community, and home settings to provide a broad range of nursing services aimed at disease prevention, treatment, and health policy implementation. They apply critical thinking and nursing process skills to screen, assess, plan, deliver and evaluate health interventions for individuals and populations. This position will include triage to assess clients' health status for immediate care, clinical appointment, education for care at home, or referrals to resources. Demonstrate knowledge-based care to assigned clients and programs in accordance with standards of practice, nursing practice act, Oregon Administrative Rules, County and department policies and procedures. May provide specialized services within specified programs and at multiple sites according to scope, license, and program standards.

This position will be located at the Lincoln Elementary School in south Corvallis.

  • Annual salary range of:
    • Health Care Professional 3: 66,539.00 - $96,233
    • Health Care Professional 5: $74,637 - $107,944
    • These positions are eligible for up to two salary raises within your first year. An Increase may be given at the end of the 6-month probationary period, depending on starting salary, and at the 1-year anniversary, as long as your performance is in good standing. Annual performance-based increases are given until the maximum of the salary range is reached.

Minimum QualificationsThe following minimum qualifications are requiredfor this position:
  • Health Care Professional 3
    • Bachelor's of Science in Nursing (BSN); AND
    • 1 year of nursing experience
Equivalency will be considered for RN and 4 years of nursing experience.

(Bachelor's in Nursing preferred)
  • Health Care Professional 5
    • Master's of Science in Nursing (MSN); AND
    • 8 years of nursing experience
Special Requirements
  • Currently licensed as a Registered Nurse
  • 6 months of experience in pediatrics
  • 6 months experience working with adult patients
  • Hiring is contingent upon the successful completion of a background check
  • Valid Oregon Driver's License or the ability to obtain
The ideal candidate will have
  • 3 or more years of triage experience with both adult and pediatrics.
  • Experience working with a care team and scheduling staff.
  • Experience in electronic medical records.
