Position Summary

Baptist Health Brookwood Hospital

Position Summary: Administers patient care in hospital or inpatient/observation settings, providing active treatment for injuries, illnesses, medical conditions, or post-intervention assessments.

Responsibilities

Essential Functions:

  1. Assess patient needs and develop/revise individualized care plans based on patient responses and progress towards expected outcomes.
  2. Respect diversity by building respectful relationships with all team members and patients.
  3. Act as a patient and family advocate.
  4. Provide age-appropriate care, demonstrating necessary knowledge and skills.
  5. Serve as preceptor, charge nurse, educator, or nurse clinician.
  6. Collaborate with medical staff and interdisciplinary teams for effective planning and management.
  7. Model professional behavior, supporting hospital and nursing goals.
  8. Understand growth and development principles, assess patient status, and interpret information to meet age-specific needs.
  9. Coordinate care and delegate tasks appropriately within the team.
  10. Document patient care accurately, including assessments, education, treatments, and safety measures, meeting all regulatory standards.
  11. Demonstrate competency in nursing skills and equipment use.
  12. Prioritize patient care according to Evidence-Based Practice Standards.
  13. Identify and resolve problems effectively.
  14. Delegate tasks based on patient needs and team member capabilities.
  15. Communicate patient condition and unit concerns clearly to the team.
  16. Provide compassionate, therapeutic environment practices.
  17. Be aware of legal issues and patients' rights.
  18. Support onboarding and ongoing education for new staff and students.
  19. Maintain regular attendance per hospital policies and standards.
  20. Comply with all organizational policies and procedures.

Hospital Care at Home Program Functions Patient Care Hub (Virtual Hospital):

Provide patient care remotely via TeleHealth, assessing needs, developing care plans, and monitoring progress. Collaborate with in-home staff and demonstrate age-appropriate care skills.

In-home Nursing:

Deliver direct patient care in the home setting, assessing needs, and contributing to care plans. Must be able to travel within designated regions, work autonomously, and use virtual technology to connect with the care team. Transport items, equipment, and medications as needed.

Qualifications

Education/Training: Graduate of an approved nursing program with required competencies.

Licensure/Certification: Current Alabama RN license or valid eNLC multistate license; BLS certification; additional certifications like ACLS, NRP, PALS, TNCC as required; valid driver's license and reliable transportation for in-home nursing duties.

Experience: Minimum 1 year of nursing experience; 2 years for Hospital Care at Home roles.
