Summary

GENERAL SUMMARY:

Responsible for managing an assigned group of patients including assessing patients nutritional status; identifying and prioritizing patients nutrition diagnoses or nutritional needs; determining and implementing evidence-based nutrition interventions; evaluating patients response to care and progress toward clinical outcomes; and educating patients/family members regarding nutrition therapy options and expected results. Participates in interdisciplinary care planning and communicates nutritional information to health care team members. The Registered Dietitian is in the Community Wellness Department and performs duties related to community wellness as assigned.

PRINCIPAL JOB FUNCTIONS:

  • Initiates assessment and reassessment for patients identified at nutritional risk, including the nutrition focused physical exam for evidence of nutrient deficiency/excess.
  • Evaluates pertinent initial and ongoing patient data to identify nutrition diagnosis and develop individualized age-appropriate nutrition treatment plan and desired outcomes.
  • Uses the nutrition care process as a framework to guide problem identification, treatment and evaluation, and comprehensive documentation of nutrition care and patient outcomes.
  • Provides education for the patient and family to meet identified patient needs according to their needs, abilities, preferences and readiness to learn.
  • Evaluates the patients response to and effectiveness of nutrition interventions and progress toward established outcomes.
  • Documents patient care and related information according to the Merrick Medical Center procedures for documentation.
  • Collaborates with team members, patients and patients family members to implement, monitor and evaluate the treatment plan to improve or maintain nutritional status.
  • Provides administrative directions for menu formulation, food preparation and service, purchasing, sanitation standards, safety practices, and personnel utilization.
  • Evaluates foods and recipes for nutritional value and appropriateness for menu.
  • Commits to self-directed professional development for knowledge, competence, and experience pertinent to work responsibilities.
  • Develops, reviews, revises, and implements appropriate departmental policies and procedures.
  • Uses quality improvement processes, programs, or outcome to help improve department operations.
  • Provides education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection. Organizes and facilitates monthly diabetic meetings with the community.
  • Assists with organization and implementation of Shape My Health, a diabetes prevention program led by the Centers for Disease Control and Prevention.
  • Assists with organization and implementation of Wellness Connections, a worksite wellness program, which includes wellness screenings and counseling sessions for participating area businesses.
  • Provides assistance with other community events as needed.
  • Performs other related projects and duties as assigned.
EDUCATION AND EXPERIENCE:

Bachelors degree in Dietetics or Food and Nutrition with post baccalaureate internship resulting in registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN). Current Licensed Medical Nutrition Therapist (LMNT) licensure from the state of Nebraska required. Prior clinical nutrition experience in specialty area preferred. Certified Diabetes Care and Education Specialist helpful.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
