The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. The Transition Qualified Intellectual Disability Professional III/ (QIDP) works under the general supervision of the Assistant Director of Programs. The Transition QIDP performs complex (senior level) work to include working with the Transition Specialist regarding monitoring and assessing persons served in a State Supported Living Center (SSLC) as they are identified for and are transitioning into a community setting. Work involves performing complex consultative work and resource, may assist with scheduling and conducting on-site training to staff, residents, LARs, and family members regarding community relocation processes, relocation planning, Community Living Option discussions, etc., consulting with the facility QIDPs regarding the IDT identification of needed supports and services for individuals referred for alternate placement, identification of barriers to alternate placement, and initialization of programming to overcome the identified barriers. Work schedule requires regular flexibility to ensure effective supervision of individual transition is successful. Works under minimal supervision with extensive latitude for the use of initiative and independent judgment. Must demonstrate a clear understanding of the state transition process and work directly with the assigned QIDP by following the protocol for the placement process. The Transition QIDP will perform every aspect of the transitioning process to ensure a successful community placement. The Transition QIDP will be directly responsible for completing all paperwork and all addendums involving an individual's transition into the community. The Transition QIDP primary function is to work closely with the IDT and the Transition Specialist to facilitate the Community Living Discharge Planning (CLDP) process for persons served in an SSLC as they are identified for and are transitioning into a community setting. Functions include but are not limited to admissions, placements, and other duties as assigned. Working with the Transition Specialist serving as a liaison between consumers, guardians, families and others with the facility to ensure that information, referral and services are provided as needed. Essential Job Functions (EJFs): Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility. Consults with the facility QIDPs, Transition Specialist and other interdisciplinary team members regarding the identification of needed supports and services for individuals referred for alternate placement. Participates in and serves as a resource to the interdisciplinary team members in planning meetings related to community placement referrals, CLOIP issues, relocation planning meetings, etc. Ensures the timely and thorough completion of required documentation within specified timelines and maintained in an orderly manner Monitors and serves as a resource to the interdisciplinary team members in scheduling and assuring needed in-services are provided to potential providers prior to individuals visiting community homes for trial visits and community placement. Works with the facility Admissions/Placement Department with the coordination of facility-sponsored community awareness and educational opportunities including: Community Provider Fairs, community tours, in-service training, etc. Works with the Transition Specialist in providing coordination of information resources for the interdisciplinary team members and assessment of information opportunities by participants. Works with the Transition Specialist and IDT to assist with the monitoring and assessment of individuals in a SSLC as they relocate into a community setting. Performs related work as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. Knowledge, Skills and Abilities (KSAs): Ability to identify and comply with all transition duties and responsibilities when assisting with individuals transitioning into the community. Knowledge of intellectual disabilities and mental illness; of rehabilitation methods; of casework principles; and community resources. Ability to work with individuals with intellectual and physical disabilities within a therapeutic/interdisciplinary environment. Ability to communicate effectively in oral and written communication, train others, maintain records, prepare concise reports. Skill in the use and care of equipment, tools and materials used in a therapeutic service activity. Functional skills and knowledge of computer, E-mail, word-processing and specialized programs. Ability to use telephones, fax, varied printers. Able to effectively handle multiple responsibilities and plan work activities involving community placement. Ability and experience in chairing IDT meetings Registrations, Licensure Requirements or Certifications: Must possess valid Texas driver's license or obtain it no later than 90 days after hire date. Applicants with an out-of-state driver's license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy. Initial Screening Criteria: At least bachelor's degree from an accredited college or university with a major in psychology, sociology, special education, rehabilitation counseling, or a related human services field. At least two (2) years of experience working directly with persons with intellectual or developmental disabilities. Experience as a QIDP or case manager. Experience haven worked as a QIDP for the past year is preferred. Experience with ICF/IDD standards.
