Speech Language Pathologist (SLP) - Home Health

This is a part-time position located in Scottsdale, AZ

The SLP Home Health implements standards of care for speech-language services and utilizes community resources to meet the needs of individuals and families at home and in the community.

Ideal Schedule: 2-3 weekdays a week, 9AM-12PM or 12PM-3PM (we are flexible)
SLP Job Duties:
Evaluate and treat speech, language, swallowing, and cognitive disorders
Create personalized treatment plans for each patient
Collaborate with patients, families, and healthcare team
Monitor progress and adjust treatment as needed
Educate patients and caregivers on therapy strategies
Maintain accurate documentation
Attend interdisciplinary meetings for coordinated care
SLP Qualifications:
Master's degree in Speech-Language Pathology
Arizona state licensure and ASHA certification (CCC)
Minimum 1 year of relevant experience
Excellent communication and interpersonal skills
Valid driver's license and reliable transportation
