Healthonomic Primary Care is seeking a dedicated and mission-driven Primary Care Physician (MD or DO) to join our full-time outpatient team.

How you can contribute to the Healthonomic team:
- Provide comprehensive outpatient care for a diverse patient population
- Diagnose, manage, and treat acute and chronic conditions
- Participate in quality improvement programs and internal audits
- Ensure accurate CPT coding and compliance with clinical standards
- Maintain timely documentation aligned with HIPAA and internal policies
- Complete CME activities and maintain all licensure and certifications
Requirements
- M.D. or D.O. with board certification (or eligibility) in Internal Medicine or Family Medicine
- Active, unrestricted Illinois medical license and DEA registration
- At least 2 years of clinical experience preferred
- Credentialed or eligible for credentialing with Advocate Physician Partners and affiliated hospitals
- Eligible to participate in Medicare, Medicaid, and commercial insurance panels
- Strong commitment to community-focused care, collaboration, and excellence
Compensation & Benefits
- Estimated Total First-Year Compensation with benefits: $298,658.20
- Annual Bonus: Performance-based bonus eligibility tied to Clinical Integration (CI) metrics
- Time Off: 23 days of PTO annually
- CME Support: Reimbursement for CME activities and required licensure/certifications
