The Primary Care Nurse Practitioner will have an exciting opportunity to provide high-quality, patient-centered care while working in a close-knit environment that values both its staff and the community it serves. As a Primary Care NP, you will report directly to the Medical Director and/or Staff Physician and will be responsible for delivering comprehensive care to our diverse patient population, including assessments, diagnoses, treatment planning, and ongoing management of various health conditions.
In this role, the Nurse Practitioner Primary Care will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the best possible care in a supportive, collaborative setting. You will conduct physical exams, interpret lab results, prescribe medications, and provide health education to patients and their families. Your expertise in managing chronic conditions, acute illnesses, and preventative care will be essential in enhancing the health outcomes of our patients. You will also be responsible for documenting patient interactions, coordinating follow-up care, and ensuring adherence to treatment plans, all while delivering care with empathy and understanding.
