Planet Healthcare is seeking a travel nurse RN Case Management for a travel nursing job in Bronx, New York.

BLS required, PRI (Patient Review Instrument) Certification, SCREEN Training Certification NY RN license required Role: Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker. Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians/team. Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med. Is this position for UR, discharge planning, or both?: Discharge planning Common types of patients & job duties: Discharge to Home, Community, or SNF. Obtaining housing, Shelter Packets for Homelessness, Hemodialysis packers, KCI referrals for wounds vacs, and more. A binder is provided as a resource. Shifts & Scheduling: Shifts M-F 8-4:30 (7.5 Hr shifts with a 1 hr lunch). Breaks are per facility policy. Management is supportive of taking a full lunch break. Weekend Requirements: Occasionally (rare- with manager approval). Caseload on weekends is different. May be asked to cover multiple units if covering a weekend or a holiday. Holiday Requirements: Occasionally (rare- with manager approval)
