Many families and young professionals live in South Bend. Also, a college scene with a long history at Notre Dame University.ROLESupervision of Medical Staff. Will also see patients in 100% outpatient clinic, providing primary care and skilled nursing facility coverage for medically complex seniors enrolled in the PACE program. Patient Load: 28 patients/day As a Geriatric Medical Director you will collaborate with leadership to improve clinical operations & outcomes Conduct initial and periodic participant assessments Schedule: Monday Friday, 8:00 AM 4:30 PM Call: 7 on-call days/month (includes weekends) telephone triage only Participate in morning meetings and care planning Develop treatment plans in collaboration with the interdisciplinary team Provide direct care and manage acute and chronic medical issues Travel locally as needed (must have valid license, insurance & transportation) EMR Truchart PACKAGESalary $208,000$241,000 (commensurate with experience)Mileage reimbursementMedical, dental vision insurance\Short+ long-term disability403(b) retirement planPaid time off
