Physician / Family Practice / Indiana / Locum or Permanent / Family Medicine Physician needed in Northeast Indiana Job

Start Date: ASAP, Ongoing until filled with permanent provider
Hours: Monday Friday 40hrs/week
Clinic hours: Monday & Wednesday 8:00am 7:00pm; Tuesday, Thursday, Friday 8:00am 5:00pm
Call schedule: Rotates, 40 Hours/week
Requirements: Medical Degree from an accredited medical school, Active IN Indiana State Medical License, CSR, and DEA license.
Board Certified or Board Eligible in Family Medicine with Peds experience
Active IN licensure or will you accept a provider that needs to acquire licensure in IN: Will accept acquired license in IN
BLS/DEA/ CSR
Skills Preferred: Pediatric experience, mostly adult
