Facility Information: Board requirement (Example EM, FM, IM): Board Certified EM or FM (or Board eligible) Life Support Certifications Required: ACLS, ATLS AND PALS (ATLS waived for EM Board certified) COVID vaccine/exemption required? Required or exemption Temp/Emergency Privileges granted?
