• 3-year employment agreement with largest clinic north of the Sacramento metropolitan area.
• 4 x 10 hr shifts per week
• No weekend coverage required
• Clinic hours are 8am - 7pm
• 22-25 patients per 10 hrs
• CA license can be expedited in as soon as one month because it is a shortage area
• Year One
• Base Compensation: $271,000
• Arrival Bonus of $50,000 at start date
• Qualifying moving expenses up to $20,000
• 15 days PTO, 8 holidays, plus 5 days CME
• $4500 per APP supervised

• Year Two " Year Three
• Base Compensation of $239,000 plus Work Relative Value Unit (WRVU)-Based Compensation Model
• WRVUs adjusted to allow four weeks out of office
• $50,000 retention bonus at end of Year Two upon good standing
• $50,000 retention bonus at end of Year Three upon good standing
• Other Compensation
• $2,500 Continuing Medical Education (CME) per calendar year prorated
• Midlevel supervision per one FTE Midlevel $4,500/year when applicable
• Malpractice 100% covered during employment
• License expense paid in full DEA expense paid in full

• Cell phone stipend $75/month, starting year 2
