Diagnoses or treats acute health care problems such as illnesses, infections and injuries through the analysis of patient's histories, symptoms, physical findings, or diagnostic information. Prescribes medication, routes, and frequencies based on patient's characteristics such as age and gender and based on efficacy, safety, and cost as legally authorized. Facilitates communication among departments to ensure continuity of care. Acts a primary resource to patient care team. Identifies, provides, develops and maintains learning resources for patient care team. Orders, performs, or interprets the results of diagnostic tests such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays). Counsels patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, and herbal remedies. Recommends interventions to modify behavior associated with health risks. Detects and responds to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women, and older adults. Educates patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances. Acts as a primary resource to patient care team and identifies, provides, develops and maintains learning resources for patient care team.
