Physical Therapist / PT

We are seeking an experienced Physical Therapist to provide innovative, community-based rehabilitation focused on real-life activities at home, work, and community settings.

•  Perform evaluations, develop and reassess treatment plans, and coordinate care with a multidisciplinary team.

•  Treat clients in natural environments like kitchens, grocery stores, and workplaces to enhance functional independence.

•  Must have a valid PT license, BLS certification, and preferably one year of experience in neurological rehab.
