Pride Health is hiring a Physical Therapist to support our client's medical facility in Stockton, CA 95204. This is a 27-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization.
Job Summary:
• Physical Therapist. 40 hours per week (5x8s, Days: 8 am to 4:30 pm, Monday to Friday)
• Bachelor's degree in physical therapy
• Valid California Physical Therapy License
• BLS (AHA)
Physical Therapist:
• Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
• Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.
• Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).
• Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
• Order, track and manage maintenance for equipment.
• Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or functional status.
Experience required:
• Five years of Progressively responsible PT experience.
• Minimum of two years' Physical Therapy experience in a clinical setting with a frail or elderly population.
• Two years' supervisory experience with demonstrated ability to mentor and develop team members.

• Weekly Travel Pay: $3000/week to $3200/week
