Title: Physical Therapist

Location: Stockton, CA & Modesto, CA

Shift: 8-hour standard (8AM to 4:30PM)

Duration: 3 Months / 13 weeks (Temp. to hire) and Direct Full-Time option also available



Job Description:

The Client Physical Therapist is responsible for providing restorative and rehabilitative Physical Therapy services to participants both at the Center and at home. Working closely with the PACE Physician, rehabilitation staff, and the interdisciplinary team (IDT), the therapist aims to maximize participant independence, restore function, and ensure safety. Additionally, they collaborate with the Occupational Therapist to efficiently manage Client's rehabilitation department and foster cross-functional relationships within the organization and with external stakeholders.


Duties & Responsibilities:

  • Assess participants' needs, preferences, and goals to develop tailored Physical Therapy care plans in IDT meetings.
  • Provide and document therapy interventions in accordance with physician orders and care plans, including:
  • Active and passive exercises
  • Muscle re-education
  • Gait and functional training for ADLs
  • Treatments using massage, heat, sound, water, light, or electricity
  • Supervise and guide Therapy Assistants in implementing Functional Maintenance Programs (FMP).
  • Train staff, participants, and caregivers on the proper use and care of therapeutic equipment.
  • Manage equipment, including ordering, tracking, and ensuring proper maintenance.
  • Monitor and report changes in participants' conditions to the IDT promptly.
  • Coordinate with IDT members to ensure smooth transitions between care settings (hospitals, skilled nursing facilities, home, etc.).
  • Support the Marketing Team in enrolling prospective participants.
  • Participate in end-of-life care coordination and support when needed.
  • Train, supervise, and mentor direct reports within the rehabilitation team.
  • Ensure compliance with all legal, regulatory, and organizational policies.
  • Maintain confidentiality and uphold privacy standards regarding participants, families, and team members.
  • Actively engage in Quality Improvement activities, staff training, and company meetings.
  • Complete all mandatory training, education, and occupational health screenings in a timely manner.
  • Demonstrate flexibility in assignments as business needs evolve.
  • Perform other duties as assigned.

Experience:

  • 2-3 years of experience in the relevant field strongly desired.

Education:

  • Bachelor's Degree in Physical Therapy. The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy required.
  • Master's Degree in Physical Therapy or Doctorate in Physical Therapy preferred

Certifications and Licenses:

  • BLS (AHA)
  • Valid CA Physical Therapist License
