APPLY NOW. We seek a dedicated and compassionate Physical Therapist, PT to join our team in a skilled nursing facility.
RESPONSIBILITIES:
Conduct comprehensive evaluations of patients to identify physical, functional, and rehabilitation needs.
Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
Monitor and document patient progress and adjust treatment plans as necessary.
Educate patients and caregivers on exercises and techniques to promote recovery and prevent future injury.
Provide hands-on therapy including exercises, manual therapy, and modalities as appropriate.
Ensure compliance with regulatory standards and company policies in all aspects of care delivery.
Participate in clinical meetings and contribute to a culture of continuous improvement.
QUALIFICATIONS:
New passer/graduates are welcome
With Skilled Nursing or Long-Term Care, experience is a plus
Previous experience as a Physical Therapists (PT) is a plus
Physical Therapy Degree from an accredited program
State licensure or eligibility for licensure as a physical therapist
Is in good standing with all regulatory agencies and licensing boards is required
Interest in working with the geriatric population
Strong knowledge of anatomy, physiology, and medical principles related to physical therapy
Excellent communication and interpersonal skills
Ability to work effectively both independently and within a team
Strong problem-solving and critical-thinking abilities
Proficiency with electronic medical record (EMR) systems is a plus
