Lakeshore Talent is in Search of a Physical Therapist to work full time within an exciting organization we are partnering with in the home care industry.


Salary: $48.12 - $56.64/hr


Schedule: 8:00 AM - 4:30 PM (Full-time, 40 hours per week) 100% onsite with some participant home visits (25% of the time, varies weekly) - Mileage reimbursement provided



Scope of Work:

  • Provide clinical physical therapy evaluations and treatment plans for program participants aged 55+
  • Integrate physical therapy treatment plans into the Plan of Care (POC) based on physician orders
  • Conduct physical therapy evaluations and screenings, ensuring ethical practice per APTA guidelines
  • Collaborate with nursing homes and assisted living staff on participant rehabilitation needs
  • Refer participants to Occupational Therapy (OT) and Speech Therapy (ST) with team approval
  • Communicate effectively with staff, family members, and caregivers regarding participant needs
  • Administer treatment, including modality therapy, therapeutic exercises, and functional mobility training
  • Provide guidance on Durable Medical Equipment (DME) and ensure proper use
  • Conduct home visits as needed and document findings in participant records
  • Participate as an active Interdisciplinary Team member and collaborate on care planning
  • Maintain compliance with InnovAge policies and state regulations
  • Educate and provide training to staff, including CNA's, Drivers, and other non-rehab personnel
  • Assist in budgeting, inventory management, and ensuring functional DME availability

Key Traits:

  • Strong assessment and triage skills
  • Required Qualifications:Bachelor's Degree in Physical Therapy
  • Completion of a physical therapy program accredited by APTA, the Committee on Allied Health Education and Accreditation, or an equivalent CMS-approved organization
  • Active Physical Therapist license issued by the state of Pennsylvania
  • Minimum of one year experience working with the frail or elderly
  • Reliable vehicle
  • CPR/BLS Certification and First Aid Certification (ARC or AHA)
  • Adaptable and flexible - must thrive in a dynamic environment
