Dedicated Physical Therapy & Wellness is a team of physical and occupational therapists who are committed to our patients. We work within an active 55+ community, Solivita, located in Kissimmee, Fl. We provide outpatient physical therapy and occupational therapy in the comfort of our patients home or using the communities amenities available to us. We strive to give our patients one on one care that surpasses the rest. We are actively looking for a dependable, passionate, and dedicated physical therapist. All our patients are located within this one community so there is minimal travel.
This is a fantastic opportunity to get in on the ground floor of a growing physical and occupational therapy company that specializes in serving geriatric patients. The ideal candidate will have a background in physical and rehabilitation medicine and some experience handling geriatric care. We are looking for a highly motivated individual who can communicate effectively with patients, has experience managing healthcare needs, and is adept at balancing the needs of patients with an efficient schedule.

Responsibilities
- Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
- Prepare and submit clinical and progress summaries based on identified and attained goals
- Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
- Guide and consult patient.
- Timely and detailed documentation
- Assess physical ability to perform daily activities and social/leisure activities.
- Provide care that is individualized to meet each patient's needs.
- Work to promote the health and well-being of patients.
- Make referrals to other health care professionals for patients who need more specialized care.
- Recommend modifications to home, work, or school environments for those with disabilities.
- Counsel patients on the need for exercise, education, and training programs to maintain or improve their health.
- Provide patient education and training in order to maximize their recovery from illness or injury.
- Administer rehabilitation for patients who have suffered trauma., Provide ongoing treatment for patients with a wide variety of medical problems.
- Record patient information.
- Explain treatment programs to patients.
Qualifications
- Licensed Physical Therapist with a Master's or Doctor's degree of an accredited physical therapy curriculum
- Certification in Physical Therapy and valid Florida state license to practice
- Currently living in the Greater Orlando area at time of application or plans to live in the area immediately.
- Owns a personal computer/tablet/phone
- Current CPR certification
- Dedication to our patients
- Excellent Communication skills both written and verbal.
- Comfortable working with patients in their homes. Make initial evaluations and plan treatment programs for patients.
