Physical Therapist

•  Full-time clinical physician lead role under CMO supervision, providing day-to-day care for residents in facilities and homebound patients
•  Responsibilities include complex and geriatric care, collaborating with and auditing nurse practitioners/physician assistants, engaging with facility teams, and assuming medical directorships as needed
•  Focus on quality outcomes, compliance, reducing avoidable transfers, and supporting growth opportunities
•  Requires MD, unrestricted license, DEA registration, board certification, 2+ years administrative healthcare leadership, and 7+ years clinical experience preferred
•  Strong communication, organizational, decision-making, and EMR skills are essential
•  Salary range: $187,000-$230,000/year
