Our programs include early intervention, therapy services, durable medical equipment loans, and special assistance programs that reach people of all ages and abilities. As we continue to grow, we are seeking a compassionate and skilled Physical Therapist (PT) to join our team. This position offers a unique opportunity to work in a meaningful setting that combines clinical care with personal connection and community impact.
Position Summary
As a Physical Therapist with RAISEtown Care Connect, you will play a key role in supporting the physical development and mobility needs of infants and toddlers (ages 0-3) through our Early Intervention program. Your responsibilities will include evaluating children's gross motor skills, designing and implementing individualized therapy plans, and working directly with families to improve outcomes in natural home or community-based settings.
This flexible position can be tailored as full-time or part-time depending on candidate availability, and may offer the opportunity to expand beyond early intervention based on community need and therapist interest.
Primary Responsibilities
- Conduct developmental evaluations and functional assessments to identify gross motor delays and other physical concerns in children under age 3
- Design and implement Individualized Family Service Plans (IFSPs) in coordination with families and other early intervention professionals
- Provide therapy sessions in natural environments, such as homes or childcare settings, to support real-life skill development
- Coach parents and caregivers on activities and strategies to reinforce therapy goals between visits
- Maintain accurate and timely documentation of assessments, progress notes, and service logs in accordance with state and agency guidelines
- Collaborate with service coordinators, speech and occupational therapists, and other team members to ensure a holistic and coordinated approach to care
- Adhere to professional and ethical standards set by the American Physical Therapy Association (APTA) and Pennsylvania licensing board
- Attend periodic team meetings, trainings, and supervision as required
- Support the mission and values of RAISEtown Care Connect in all professional activities
Qualifications
- Graduate of an accredited physical therapy program (Bachelor's, Master's, or Doctorate in Physical Therapy)
- Current licensure to practice physical therapy in the state of Pennsylvania
- Pediatric experience, preferably in early intervention or a similar home-based setting
- Strong interpersonal and communication skills, with a family-centered and collaborative approach
- Ability to work independently and manage a flexible schedule
- Reliable transportation and willingness to travel throughout Huntingdon County
- Ability to lift, move, or assist children in therapy when needed
- Proficiency in documenting electronically and meeting state and program deadlines
Preferred Qualifications
- Knowledge of the Early Intervention system and Pennsylvania's Infant/Toddler program
- Understanding of typical and atypical child development
- Familiarity with IDEA Part C, IFSPs, and family-centered care principles
- CPR/First Aid certification
- Bilingual (Spanish or ASL) a plus but not required
