Position Overview: Physical Therapist (PT)

As a Physical Therapist at Pinnacle Home Care, you'll play a vital role in delivering high-quality care to patients in the comfort of their homes. You'll work under the direction of a physician and nursing supervisor, ensuring care meets all regulatory and clinical practice standards.

Key Responsibilities
Perform initial and ongoing patient assessments, including OASIS documentation.
Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.
Collaborate with the interdisciplinary care team to optimize patient outcomes.
Provide education and support to patients and families on therapy programs.
Supervise Physical Therapist Assistants every 30 days.
Communicate patient progress and changes with physicians and care teams.
Document all visits and progress notes on the same day of service.

Qualifications
Graduation from an accredited Physical Therapy program.
Active PT license in Florida.
Current BLS certification.
Valid driver's license and reliable, insured vehicle.
At least one year of PT experience (home health experience preferred).
Proficiency with electronic medical records is a plus.
Physical capability to perform required tasks (standing, bending, lifting).
Strong self-management, communication, and judgment skills.
