We are seeking a Physical Therapist to join our home health team in Portage, WI. The role involves providing physical therapy to patients of all ages, evaluating their needs, establishing treatment plans, and supervising treatment modalities. Responsibilities include patient and family education and documentation of services.
Full-time position, 80 hours per pay period, day shift. Requires a minimum of a bachelor s degree in physical therapy, Wisconsin/Michigan licensure, and current BLS certification.
