Description:

AHC Home Health and Hospice of Sacramento is looking to add a full-time Physical Therapy Assistant (PTA) to our Home Health team. In this role, you will provide physical therapy services to clients in their homes in accordance with the Plan of Care and Agency policies and procedures. The PTA performs functions that require substantially specialized knowledge, judgment, and skill.

Qualifications

Graduate of Physical Therapy Assistant program.
Possesses current registration or license as required by the state of practice.
Minimum of two (2) years experience in a health care facility or equivalent experience preferred.
Demonstrates good written and verbal communication skills and interpersonal skills.
Licensed driver with automobile insured in accordance with state/Agency requirements. Good driving record and reliable transportation.
Current CPR certification.

Essential Job Functions

Performs physical therapy treatments for those receiving services under a medically approved Plan of Care.
Follows the therapy care plan to provide direction to other home health team members involved in the Plan of Care. Reviews and updates are per agency policy and client needs.
Documents all findings, plans, interventions, and client progress (outcomes) on designed forms and submit per Agency guidelines.
Communicates plans and changes to the Case Manager and other Agency staff through the care plan, written progress notes, and participation in care conferences.
Implements teaching appropriate to the client's needs and within the context of his/her environment and learning ability.
Plans, teaches, supervises, and counsels clients about their physical therapy needs to promote improvement or recovery, symptom management of disease process, and community resources.
Demonstrates knowledge of safety/infection control practices by compliance with agency policies and procedures.
Assesses safety of the environment and takes the initiative to help prevent accidents and promote safety.

Physical Requirements

Requires a full range of body motion including the handling and lifting of patients.
Manual and finger dexterity and eye-hand coordination required.
Occasionally, will lift and carry items weighing up to 50 pounds or greater.
Requires corrected vision and hearing to normal range.
May require working under stressful conditions or working irregular hours. Involves exposure to communicable diseases.
Requires a current driver's license and the ability to drive and access a patient's home without an assistive device.
