Overview:
We are seeking a dedicated and skilled Physical Therapy Assistant to join our team at skilled nursing/Assisted Living facility that offers both long-term and short-term rehabilitation services.

Responsibilities
Assist licensed physical therapists in implementing treatment plans for patients in various settings, including outpatient clinics and skilled nursing facilities.
Provide direct patient care by administering therapeutic exercises and modalities.
Monitor patient progress and document treatment outcomes.
Educate patients and their families on care plans and home health strategies.
Collaborate with healthcare teams to ensure comprehensive patient care and effective discharge planning.

Qualifications:
To be considered for this role, candidates must meet the following requirements:
1. Graduate of an accredited Physical Therapist Assistant Program certified by the American Physical Therapy Association.
Licensed and/or eligible for licensure as required by the state of practice.
