Description: Supports the Trinity Health Alliance of Michigan pharmacy team to enhance workflow for clinical pharmacy services throughout the network, including medication therapy management (i.e. medication reviews and chronic disease management) and medication adherence. Provides patient identification, outreach and scheduling support for Pharmacists completing clinical services within the network and providing telephonic patient care (i.e. pre-visit med rec, adherence monitoring) and supporting data/metric tracking, trending and reporting. The Population Health Pharmacy technician supports the Trinity Health Alliance of Michigan pharmacy team to enhance workflow for clinical pharmacy services throughout the network.
Primary actives include outreach and scheduling for comprehensive medication management services with network pharmacists & outreaching to patients to educate and support them with medication adherence.
This is a remote position, but the ideal candidate will be located in Michigan. Education details Credentials/Licensure Required: Michigan Board of Pharmacy Technician registration/license; Certified Pharmacy Technician (CPhT) with PTCB.
Preferred: Graduate of formal pharmacy technician training program; BLS certification.
Related Experience
Required: Three years of pharmacy technician practice.
Preferred: Experience with patient outreach programs. Sales related experience.
Other Knowledge, Skills and Abilities Required: Ability to communicate in English (verbal and written). Ability to work independently and collaboratively with staff.
Preferred: Good knowledge of the managed care environment. Good knowledge of retail or specialty pharmacy practice.
Computer Competency Familiarity with standard desktop and Windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.
Physical/Mental Ability to use a computer keyboard. Sitting, walking or standing for long periods of time. Ability to work with frequent interruptions and meet aggressive deadlines.
ESSENTIAL FUNCTIONS - Interacts with all levels of medical staff to discuss relevant patient information as warranted.
- Conducts chart review and patient interviews to identify patient specific reasons for poor medication adherence; resolves identified problem or escalates to clinical pharmacist.
- Performs medication history taking for patients recently discharged from the hospital or as requested by the Clinical Pharmacist. Records medication information in the electronic health record and updates existing information as needed to ensure accuracy of the patient home medication list.
- Gathers data and generates reports to improve clinical pharmacy quality metrics and pharmaceutical cost.
- Documents patient cases in electronic medical record effectively.
- Appropriately identifies situations for referral to Clinical Pharmacist (i.e. medication adverse event, medication ineffective, drug-drug interaction, lab test indicated).
- Provides education to reinforce patients' self-management of medications, including proper administration, medication adherence, storage and drug disposal.
- Prepares and maintains patient education materials/handouts.
- Proactively contacts patients identified from medication adherence-related reports, high-risk patient reports or Medication Therapy Management (MTM) eligible patient reports.
- Participates in training and orientation of new employees as needed.
- Reports issues and concerns to manager in a timely manner.
- Performs other related duties as assigned.
