General Summary
Level I Pharmacists provide prescribed pharmaceuticals and drug information as needed for patient care according to professional standards and practices. The Level I Pharmacist role includes routine distributive and clinical services with time in those domains contingent upon the needs of the facility and department. Level 1 Pharmacists may advance to a Level II Pharmacist position based upon job performance and successful completion of the requirements for advancement established in the Pharmacist Career Ladder Program (PCLP) criteria grids.
Essential Functions:
Maintains the requirements located in the PCLP Mandatory and Optional Criteria Grids for the Level I Pharmacist.
Provides oversight of safe, accurate, and timely medication order verification including clarification of any medication orders with discrepancies prior to dispensing.
Provides pharmacy management and oversight of:
• Integrity and security of medications in all care locations.
• Investigational drugs.
• Preparation of injectable chemotherapy.
• Medication wastage, returns, and recalls.
• Reconciliation of controlled substance wastage.
• Compounding of formulations.
• Automated dispensing cabinets.
• Medication label components and label quality.
Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the facility's available protocols which may include:
• Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up).
• Total parenteral nutrition (TPN) consults.
• Anticoagulation initiation and follow-up pursuant to a consult.
• Renal dose adjustments.
• Intravenous to oral conversions.
Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.
Provides medication event prevention, monitoring, and reporting with an increased emphasis on patient safety.
Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications.
Adheres to WellSpan Health's medication-use policies and standard work.
Common Expectations:
Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment.
Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
Maintains established policies and procedures, patient experience caring behaviors, objectives, quality assessment, safety, environmental and infection control standards.
Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
Assists in preparing required reports by compiling information.

Qualifications
Minimum Education:
Bachelors Degree Pharmacy Bachelor's Degree. Required
Work Experience:
Less than 1 year Hospital experience. Required
Licenses:
Licensed Registered Pharmacist Upon Hire Required
Courses and Training:
Completion of a pharmacy program from accredited school of pharmacy. Upon Hire Required
